This program is an Eastern Ontario region project of the Alzheimer Societies (Cornwall, Ottawa and Renfrew County), in collaboration with the Champlain Community Care Access Centre, the Ontario Association of Non-Profit Homes and Services for Seniors (OAHNSS), the Ontario Long Term Care Association (OLTCA), the Ontario Hospital Association (OHA), the Ontario Ministry of Health and Long-Term Care (MOHLTC), and the Ontario Community Support Association (OCSA). The program focuses on reducing the personal and social consequences for a senior who is transitioning into long term care, as well as for the family who are transitioning from full time caregiving to becoming a member of the long term care health team.  Educational resources including printed material and workshop sessions have been developed and are available in Renfrew County, Ottawa and Eastern Counties.
