Community Care Access Centre/In Home Services

Community Care Access Centres (CCAC) are not-for-profit community agencies, which are responsible for the direct provision of information and referral services, placement coordination services and case management. On behalf of individuals in the community, they are also responsible for the indirect delivery (through contracts) of: professional services such as nursing, physiotherapy, occupational therapy, social work, nutrition therapy, speech and language pathology; personal care support with activities of daily living (e.g. bathing, transferring, dressing) and respite. CCACs are funded by the Ministry of Health and Long-Term Care and governed by government-appointed Boards of Directors.
